ENROLMENT FORMS

To enrol at Malvern Springs Primary School, please make sure you have completed the Application for Enrolment Form.

You will also need to provide:

  • proof of address (utility bill/rates notice)

  • an updated Immunisation History Statement (downloaded from Medicare) 

  • your child's original Birth Certificate
